Passenger Information Forms

In order to be fully compliant with certain foreign government and airline security regulations, Passenger Information Forms are required for all Haimark cruises. Each passenger must fully complete the form needed for his/her cruise. Forms are included in your confirmation materials, but may also be downloaded from the links below and return to Haimark at your earliest convenience. Completed forms must be received by Haimark before final documents will be sent. 

The Passenger Information Forms included below are in .pdf format and can be saved to your printer-connected device, printed, and completed by hand, or opened online, completed on your computer, and printed.
